If you have a long-term medical condition and take medications regularly, these will be added to your medical record as repeat prescriptions. You can request them by completing the right-hand-side of the prescription. Ideally these should be requested 48 hours in advance by post or by using our online request link above.

Please also select a local pharmacy for your prescription to be sent to, and allow at least 48 hours before attending your nominated Pharmacy before collecting your prescription. Some pharmacies offer a ‘managed prescription’ service. If you choose this option then you do not require to contact the practice for ordering or collection of prescriptions.

Please complete the form above to request your prescription online.

What to do with old medicines

Take it to the pharmacy you got it from or bring it in to the surgery. Do not put it in your household bin or flush it down the toilet.
