Join the surgery

Before registering with us please check if your address is within our catchment area. Our Practice boundary covers mainly the EH4 area, of which a boundary map can be found opposite.

If you are not within our Practice area please visit Scotland’s Service Directory – Find health services in Scotland | NHS inform to locate your nearest Practice. You can also contact NHS Inform on 0800 22 44 88.

To register with our Practice you must completed both a GPR Registration form as well as the relevant lifestyle questionnaire. These can be collected from the reception or can be downloaded on the links below. Once complete these should then be either handed in, posted or emailed back to us at 5 Quality Street, Edinburgh EH4 5BP or Reception.s70709@nhs.scot Once these are received and processed you will be added to our Practice list within 2 weeks. We don’t offer routine new patient appointments however if you require repeat medication or have any concerns please speak with our Reception team to make an appropriate appointment.
